While you are on the ward, a full assessment of your needs will be completed.
Within 48 hours of being admitted, we will discuss with you an estimated date of discharge (known as an EDD).
This gives everyone a date to aim for in terms of being able to discharge you home or to another place of care. It will be regularly reviewed. Most patients admitted to Clifton will be here for no more than 28 days.
Staff on the ward that are allocated to supporting your care will develop a personalised plan of care and identified goals you wish to achieve with you.
These will be reviewed on a regular basis by the multi-disciplinary team to assess your progress towards them. All members of the ward team will follow this plan of care to help you achieve your goals.

When you are admitted to a ward at Clifton Hospital an advanced practitioner or doctor will write your prescription for you and your medications will be obtained from the hospital pharmacy at Blackpool Victoria Hospital. Registered nurses will issue your medication to you as prescribed throughout the day.
Registered nurses are not allowed to issue any medication to you that has not been prescribed.
If you are prescribed anything different while you are at Clifton Hospital, your GP will be informed when you are discharged so this can be added to your repeat prescription for the next time you order your tablets when you are at home
