We are in the process of adding each as a webpage with the option of a PDF download as not all of the current leaflets are in an accessible format. Read our accessibility statement to learn more. 

As part of your care at our Trust, you should be given information to support your visit, appointments, and treatment. You can access the patient leaflets we have available electronically below.

If you have any feedback on our leaflets, please contact the Patient Information Officer. If you would like to become involved in improving our patient information leaflets while they're in draft form, you can apply to join our Trust Readers' Panel. The Readers' Panel is a voluntary group made up of members of the public and patients who read up to one or two leaflet drafts per month and feed back on if they're understandable, helpful, and accessible for patients. Changes can then be made to the drafts based on this feedback. If you are interested in joining the Reader's Panel, please complete the Reader's Panel application form.
