Intermediate care is a multidisciplinary service that offers a patient centred approach by integration of health and social care. It helps people to be as independent as possible by providing support and rehabilitation to people at risk of hospital admission or who have been in hospital. It also aims to ensure people transfer from hospital to community in a timely way to prevent unnecessary admissions to hospital and residential care.
The Intermediate Care team offers Physiotherapy and Occupational Therapy for individuals in their home who have a range of health conditions affecting their ability to carry out essential day to day activities.
Our Aim
Our aim is to encourage people to regain independence and confidence to take part in daily activities such as:
• Walking
• Getting on and off the bed/chair/toilet
• Washing
• Dressing
• Cooking
• Getting in and out of the house
• Taking part in interests and hobbies • Accessing the local community
• A referral to our team will have been received through social services or hospital discharge team
• A member of the team will contact you to arrange an appointment to see you in your home
• At the appointment an assessment will be carried out with you where any difficulties you are experiencing can be discussed and observed
• If we can help you to overcome the difficulties you are experiencing a rehabilitation programme will be developed with you
• The rehabilitation programme can include tasks/exercises to do independently or alongside the trained reablement support workers
• Your programme will be reviewed and updated and once your reablement episode is complete you will be discharged from the service or transferred to another service if there are ongoing rehabilitation needs
Rehabilitation goals will be agreed with you which you will need to follow in order to progress. These may include:
• Practice of day to day activities
• Provision of walking aids
• Home exercises for strength, balance and posture
• Provision of equipment to assist in the rehabilitation programme
