If you are over the age of 50, have had a fall in the last 12 months and registered with a Fylde & Wyre GP then you may meet the criteria for a referral to the falls service.
If the fall has occurred as a result of a dizzy spell, blacking out, underlying infection or long-term medical condition a GP may need to further investigate.
Our Specialist Falls Team is made up of:

Speak to your Doctor, Health Care Professional or Social Services to assess whether you meet our criteria and request a referral to our team.
