Who are the Early Supported Discharge Team?

The Early Supported Discharge Team (ESDT) are a group of healthcare professionals who provide intensive rehabilitation in your own home for up to six weeks.

The team consist of physiotherapists, staff nurses, technical instructors, administrators and occupational therapists. We support people who have a GP within the Blackpool, Fylde and Wyre area.

The team will help you to return back to being able to do those things which you were able to do prior to becoming unwell. For example they can help increase your strength so that you can walk further, stand longer. They can help you to regain your confidence in carrying out daily activities such as showering or shopping.

No, the rehabilitation provided by the team is part of the NHS service and is free at the point of care.

A member of the team will visit you within 24hrs of being discharged from hospital. They will talk to you about the things you want to do again and set some goals with you. Although the team do not provide care they will consult with your carers / family / friends and show them the exercises and activities that you are doing to support you in your rehabilitation.

Every effort will be made to ensure that the goals you have set at the beginning of the rehabilitation programme are achievable. The team can also refer you to other services who may be required to support you with ongoing care.
