The ADAS office is located from the Main Hospital Entrance up the escalators/ lift onto the main corridor then take the second corridor to the left. The nearest car park to the ADAS department is the Multi - storey at the Main Entrance.

Your first visit

All patients referred to the service are seen for the first time at the ADAS office at Victoria Hospital or alternatively at one of the outreach clinic centres for a blood test. This will be followed by a New Patient Education Session either face to face or by telephone. At this session you will be provided with information regarding ADAS and how the service operates, the reason for and duration of your new treatment and an explanation of how Warfarin works and how it is monitored.

A blood sample will be taken from your thumb and tested. The result of this test (the International Normalised Ratio test) will then determine the dose of Warfarin you will be asked to take until your next appointment.

What to bring with you

At all your clinic visits you are asked to bring with you the following:

• your yellow Anticogulation Therapy Record book

• details of any medications and/or any medication changes since your last visit

• dates of any other known appointments, holidays booked or planned time away from home.

It is very important that you make every effort to attend for your appointments but if you do need to reschedule please ring on 01253 956719 between 09:30-11:50 or 13:10-16:30, Monday to Friday. If your call is unanswered or you telephone outside these hours, please leave a message and we will ring you back as soon as possible.

Please do not arrive more than 10 minutes before your appointment time. At the reception desk you will be asked a number of questions relating to your Warfarin treatment. You will then be called into the testing area where you will have a blood sample taken from your thumb. You will then be asked to retake your seat in the waiting area.

Once your Warfarin dose has been calculated you will have your yellow therapy record book returned and as a safety measure, you will be asked to confirm your name and date of birth. Your new Warfarin dose and next appointment date will be confirmed with you before you leave the clinic. Depending upon how busy the clinic is, it is anticipated that from your appointment time your visit should last no longer than 30 minutes.

Please note: ADAS clinics operate Monday-Saturday at 10 outreach sites across the Fylde coast as well as on site at Victoria Hospital. Attendance is strictly by appointment only.

Rarely, some patients may experience unusual bleeding and/ or bruising and should this occur you must contact the ADAS office and arrange to have your INR tested. If your symptoms are severe you are advised to contact your G.P. or ‘Out of Hours’ service/ attend A&E as soon as possible – remember to take your yellow therapy record book with you.

Please note: FAILURE TO MONITOR YOUR WARFARIN THERAPY CORRECTLY CAN BE EXTREMELY DANGEROUS.
