Blackpool Victoria Hospital (BVH) is a Major Trauma Unit that is part of a wider major trauma network serving the surrounding areas. Our Major Trauma service is designed to deliver high quality specialist treatment and rehabilitation to patients of all ages. You may be transferred in or out of a local Major Trauma Centre (MTC) after initial treatment or start your journey here at BVH. Our network MTC is Royal Preston Hospital.
During your stay you may meet our Major Trauma Rehabilitation Coordinator who can talk through any onward rehabilitation needs and support you and your family through these decisions. They will work to ensure you are aware of your plan of care and what to expect throughout your hospital stay and on discharge.
It is common on the first day of admission for the multi-disciplinary team (MDT) to start planning for your discharge.
You will be assessed by various health professionals during your admission. These may include physiotherapists, occupational therapists, speech and language therapists and dietitians. Together, we will support you to identify your therapy goals and a management plan.
All assessments take a team approach, involving you, your family or any key people close to you and the MDT. The Major Trauma Rehabilitation Coordinator will work as a link between the medical team, nursing team and therapy teams to ensure you feel fully supported.
If it is identified that you require either support to go home or further rehabilitation, you will be referred to the Transfer of Care team.
There are several options for discharge once you are admitted to the acute hospital.
The Transfer of Care team is an integrated team of Health and Social Care Professionals who will work with you, your family and the MDT to identify the most appropriate discharge pathway for you. If it has been identified that you are safe to go home, the team will discuss and arrange the support you may need, such as a package of care. They will also consider any other services that might be of benefit to you, such as ongoing community rehabilitation. If, however, it is felt that you need further rehabilitation before returning home, the team will consider a period of inpatient rehabilitation at a facility appropriate for your needs.
You will be given a copy of your rehabilitation prescription whilst in hospital. This document will be started on admission to hospital and updated during your stay. It outlines your rehabilitation needs and goals to optimise your recovery.
Once you are optimised for discharge from hospital, you may be supported in the community for any further follow up appointments.
These appointments may include district nursing, therapy teams, outpatient fracture clinics or consultant follow up clinics based on your needs. Should you have any questions or concerns regarding follow up, please contact the Major Trauma Rehabilitation Coordinator who will be able to advise/sign post you to the most suitable service to support you.
