The Assessment and Rehabilitation Centre (ARC) is situated in Blackpool and offers enhanced clinical rehabilitation and social care rehabilitation pathways for people who need a short period of intensive support in a 24 hour setting.
Support is accessed via an assessment of need from an adult social care or health professional. ARC accepts patients on step down pathways for patients transferred from acute-based care or step up pathways for patients requiring an escalation of need from home and to prevent admission to hospital.
Both social care and health professionals work to provide individually tailored support programmes for up to six weeks to help people maximise their independence.
The team is made up of rehabilitation support workers, occupational therapists, physiotherapists, nurses, GPs, and social workers who all work together to identify goals to promote independence.
