We accept referrals from health professionals and Blackpool/Lancashire Special Educational Needs and Disability team only.
It would be useful to liaise with a child's nursery for further information to support any referral. You can access our referral form on EMIS as a letter or by the word document which has been supplied to all the health visiting teams.

The referral will then be triaged by the multidisciplinary team and a timeframe allocated for an appointment. Some children will need to be seen urgently and this is decided at triage so please include as much detail as possible about the child's strengths and areas of need.
