Presentation’s indicative of needing CYP Learning Disability Team Support:
  • The child must have a learning disability as defined in Section 2.1 which is moderate and severe
  • Must be registered with a Blackpool CCG Member GP Practice
  • Up to 18 years old
  • Children and young people with a Learning disability ranging from moderate to severe who may present with behaviours that challenge, where sustained attempts have been made, but have failed, to manage the health needs of the service user without improvement (i.e., an initial phase of care has been delivered without full improvement, and on-going specialist healthcare and treatment is required)
  • Have a diagnosed moderate to severe learning disability which may include co-morbidities relating to different neurotypes and/or issues relating to emotional wellbeing which is causing some impairment on functioning i.e., managing day to day activities. This may also cause impact on complex social issues, such as housing or substance misuse issues
  • Comorbid presentations relating to emotional wellbeing may be supported by the link workers within CAMHS who specialise in Learning Disabilities and Autism. These young people are not to be discharged from CAMHS/ Youtherapy whilst being joint worked by CYPLDT
