Please note we are accepting all versions of the referral form (paper, PDF and online version) until 31 August.
From 1 September 2024, we will only accept online referrals. If there is a reason you are unable to complete the online referral form after 1 September, please contact the paediatric therapy admin team on 01253 951101 or email bfwh.
If you are concerned about a child's speech, language or communication for a child who is under 5 years old in Blackpool, please contact your health visitor or pre-school setting to discuss a referral.
Referrals for this cohort of children are discussed at the WellComm triage panel therefore are referred via a different route and referral form.
