Enhanced primary care (EPC) is a model of care delivery which takes a multi-disciplinary team approach to help people to manage their own care and live independently in their own homes and community.

The focus is on prevention and not just treatment, with personalised care and goal setting around patients’ needs and wellbeing, integrated multi-disciplinary working and support for patients to self-care, self-manage and improve their health and wellbeing.

EPC can support patients aged 18 and over registered with a Blackpool, Fylde or Wyre GP, and have either a long-term condition which they require support to manage, and/or are struggling to cope and self-manage.

This can include those who require additional support to access services, those living with a carer who appears under pressure, those who require assistance with social isolation etc.

EPC also includes the District Nursing Service which provides a range of high quality and effective nursing care to people in their own homes, care homes and treatment rooms.

For patients registered with a Blackpool GP, there are six neighbourhood teams which work collaboratively with the GP practices. These neighbourhood teams include therapy staff, community nurses and matrons, health and  wellbeing support workers, social workers, mental health nurses, neighbourhood assistants andadministration staff.

The neighbourhood multi-disciplinary teams support people in the community allowing them to stay in their own homes wherever possible rather than be admitted to hospital.

For those patients registered with a Fylde or Wyre GP, care is delivered by a team of integrated care coordinators, health and wellbeing workers, neighbourhood assistants, administrators, assistant practitioners, community matrons and community nurses.
