What we do
The Early Supported Discharge (ESD) Team is a rehabilitation service for individuals within the Blackpool, Fylde and Wyre areas.
Patients are referred into the ESD service by Therapists within both Blackpool Victoria and Clifton Hospitals. The ESDT consists of a variety of Therapists. The service is for adults who are medically stable but have the potential to return to their previous level of independence/function.
