A hospital passport is a document about you and your health needs. This document is designed to be downloaded and printed off to be completed.
It also has other useful information, such as your interests, likes, dislikes, how you communicate and any reasonable adjustments that you might need.
We advise that all people with learning disabilities are offered a hospital passport, the completion of this is supported by family, carers and staff. We also advise that this is kept up to date and revisited on every admission to identify any changes.
